Forms
There are a number of forms you will need to obtain and fill out in order to be registered as a student at Georgetown. This page will provide you with links to as many of these forms as possible. Try to get them completed before the start of school; it will save you a lot of running around once you get to campus.
Student Health Center
- Immunization Certificate
- District of Columbia law requires every student under the age of 26 to provide proof of immunizations. You can download and print the needed form; you can also call 202-3100 and request to have a printed copy of this form sent to you.
Health Insurance
- Student Access+ Online Acceptance/Waiver
- All full-time students at Georgetown University are required to have health insurance. A charge for student health insurance (Georgetown University Sponsored Plan) will AUTOMATICALLY be charged to your student account. If you are already insured by another plan, and will continue that insurance for the entire school year, you must waive the University insurance to have it taken off of your bill.
